Merck is localizing its diabetes blockbusters in Saudi Arabia.

The official framing is Vision 2030 localization. True. But look at what's happening to these molecules in-market:

📉 Branded Januvia's share collapsed from ~99% to ~56% in four years — with volume down ~46% in a single year as generic sitagliptin flooded in (now ~1.76M units and roughly half the market by value).

📉 Janumet is in structural decline — units down ~9%/yr, and −12% last year alone.

📈 Meanwhile the molecule itself is booming — total sitagliptin demand is up sharply. The pie is growing; MSD's branded slice is shrinking.

So here's the sharper read, from a few angles:

1. Localization as defense, not just policy.

When your patented franchise is losing half its volume to generics, "Made in Saudi Arabia" isn't a concession — it's a shield. Local content protects tender eligibility and procurement preference exactly when generics are trying to displace you. MSD is manufacturing locally to stay on the formulary.

2. The procurement lever cuts both ways.

Saudi Arabia ties market access to local content — and a genericizing originator suddenly has every reason to comply. The Kingdom gets its factory and its tech transfer; the multinational gets a defensible position. Aligned incentives, born of pressure.

3. The generic wave is the real headline.

The untold story in this data isn't the localization — it's how fast a global blockbuster gets commoditized once the molecule opens up. Four years from 99% to 56%. That's the speed of the patent cliff in this region.

The lesson: in Saudi pharma, localization and lifecycle strategy are becoming the same decision. The smartest originators aren't localizing their growth products — they're localizing the ones under generic siege, to defend the shelf space they have left.

Note: figures are market-level unit/value data for sitagliptin products; the localization itself is a commitment under LCGPA's Economic Participation Program, not a disclosed commercial contract.
